Photography & Video Consent Form

I understand that:

my images will be held in accordance with the General Data Protection Regulation and the Data Protection Act 2018;
my image will be held for the existence of the Network, for promotional purposes online, unless I withdraw my consent;

I can withdraw my consent at any time by emailing lay.chaplain@english.op.org.

NAME

I hereby consent to the photographs and/or video recordings and identifying personal data also being used in other Network-wide marketing and promotional communications, including on the Network website, in Network social media, and on online marketing materials.

I hereby consent to the photographs and/or video recordings being used by Network-associated third parties, on their websites, social media channels or elsewhere in their online marketing materials.
